What is AFC in Massachusetts?

The Adult Family Care (AFC) Program, also known as Adult Foster Care, is designed to help people on MassHealth age 16 or older who cannot safely live alone due to medical, physical, cognitive, or psychiatric difficulties.

Who can be an Adult Family Care Caregiver?

Caregivers must be able to devote the appropriate time necessary to provide needed personal care to the participant to ensure safety and well-being at all times.

The caregiver can be a family member except for the member’s legal guardian, spouse, or non-family member who has time in their lives and room in their home and is willing to provide assistance and supervision. Caregivers must be thoroughly screened and complete an application process.

Who is eligible for Adult Family Care in Massachusetts?

  • Adults who are unable to live alone because of a medical, physical, cognitive, or mental disability
  • Adults who require daily assistance with one or more activities of daily living (ADLs)
  • Adults who are eligible for the MassHealth standard.
  • Must receive a referral and order for Adult Foster Care from their Primary Care Physician (PCP).
  • Intake Forms from Caridad Independent Living, LLC.

Our staff members come from a wide range of backgrounds and speak many languages, including English, French, Haitian Creole, Cape Verdean Creole, and Spanish.

We provide support to caregivers and their loved ones through:

  • A tax-free stipend
  • Regular visits from a registered nurse and a case manager
  • Being a listening ear
  • Referrals to services and community resources
  • Connections to other programs and services at Caridad

REQUIREMENTS FOR THOSE IN NEED OF CARE:

Are you unable to live alone due to a medical or mental condition?
Is there a friend or family member who can take care of you?
Do you have a medical or mental condition that prevents you from living alone?
Do you have MassHealth?
Do you live with your caregiver, or are you willing to move in with your caregiver? (Caregiver cannot be your spouse or legal guardian.)
Do you require supervision or physical assistance with at least one of the following daily activities: walking, bathing, dressing, toileting, or getting up from or down to a chair, couch, bed, toilet, or bath?
